Select “Councils New Online Forms”

The User name  (If you do not have this information, please asked your District Deputy)

The Password  (If you do not have this information, please asked your District Deputy)

Click on the Login button

In the Enter New Forms column click on the target logo of the activity form you wish to create

Select Council Number from the drop down and click submit

You will get the interactive program form for the Activity displayed on your screen.

 

To SAVE, PRINT  or send the program activity forms after you have log into under Council New Online Forms:

Under Council online forms

Select the program activity

Select review submitted form,

Select the council number and click the submit button

Select the record you wish to view, save, print or send

Select the Printer friendly option to Print

At the top of the Screen, select File menu option, select  "Save As" or "Send To" menu option and follow menu options.

Printer Friendly options are available for degree schedule selections.

 

Please note the following:

Passwords expire ever 30 day’s, just re-key them

Photo’s submitted are limited to 250 K

Date is entered using the Calendar Icon next to the entry block
